Durable Power of Attorney (Effective Immediately) – Allows an individual (called the “Principal” or “Grantor”) to authorize someone else (called the “Attorney-in-fact” or “Agent”) to act on his or her behalf. This Durable Power of Attorney document becomes effective immediately upon signing and stays in effect even if the Principal later becomes incapacitated or disabled.

This particular Form becomes effective immediately and remains in full force and effect even if the Principal (i.e. the Grantor) later becomes incapacitated.
